Furnizare echipamente protectie spital Furnizare echipamente protectie spital; Cantitatea estimata prevazuta in documentatie se va achizitiona in functie de stocuri, solicitate sectii si buget alocat; Oferta financiara se va calcula in functie de cantitatea maxima (cantitate referate) din tabelul caietului de sarcini si se va incadra in valoarea fiecarui lot in parte: -Lot 1-bonete uf-25000 lei fara tva; Lot 2-masti chirurgicale 3 straturi-50000 lei fara tva; Lot 3-papuci uf-15000 lei fara tva; Lot 4-sorturi pvc-30000 lei fara tva; Lot 5- halat ramforsat-32000 lei fara tva; Lot 6- halat vizitator-117000 lei fara tva Operatorii vor putea depune oferta pentru unul sau mai multe loturi, cu conditia incadrarii in valoarea estimata a fiecarui lot si valoarea estimata totala. Operatorii economici vor depune oferta integrala in plic inchis la secretariatul spitalului pana la data de 10.02.2026 ora 14:00 cu toate documentele solicitate in documentatie si in anunt, inclusiv cu mostra din fiecare produs ofertat in parte; Oferta va cuprinde urmatoarele: -declaratiile pe propria raspundere privind neincadrarea in art 164, 165, 167 din Legea 98/2016 si declaratie privind respectarea art 51 din Legea 98/2016; -certificat constatator , valabil la data depunerii ofertei; -oferta tehnica care cuprinde formularul de oferta solicitat in documentatie si documente solicitate in caietul de sarcini: aviz de functionare-la produsele de la lotul 2 si 5 , certificat de conformitate CE de la producator , fise tehnice- cu fotografii, de la producator, documente din care sa reiasa aplicabilitatea in spital, buletine de incercare; -formular oferta financiara, cu centralizator preturi, cu evidentierea fiecarui lot ofertat, in lei fara tva, cu maxim 2 zecimale; -declaratie privind insusirea clauzelor contractuale; -declaratie cu informatii generale despre operator. -Transport gratuit in incinta magaziei spitalului; -Plata cu OP la 60 zile prin Trezorerie Operatorii vor depune oferta integrala in plic inchis si nu se vor accepta ofertele incomplete; Nu se accepta oferte trimise pe email si nici oferte alternative; Informatii suplimentare : dna asistent sef spital 0368202012 si solicitati interiorul catre aceasta dna.
