Medicamente pentru aparatul de urgență Medicamente pentru aparatul de urgență Scrisoare de înaintare- Formular nr. 1; Certificat de înregistrare sau Certificat Constatator eliberat de Ministerul Justiției Oficiul Registrului Comerţului din care să reiasă că operatorul economic este legal constituit, că nu se află în niciuna dintre situațiile de anulare a constituirii precum și faptul că are capacitatea profesională fiind autorizat să realizeze activitățile care fac obiectul contractului; Propunerea tehnică - Declaraţie de consimţământ privind prelucrarea datelor cu caracter personal Formular nr.2; Propunerea financiară conținând Formularul de ofertă – Formular nr. 3 completat Oferta va fi valabilă 60 zile de la transmiterea acesteia Termenul de valabilitate al produselor ofertate să fie de minim 12 luni de la livrare. Achizitorul are dreptul de a inspecta produsele pentru a verifica conformitatea lor cu specificațiile prezentate. Ambalajele de transport trebuie să asigure păstrarea integrității și calității produselor pe timpul transportului, ele trebuie să fie curate, uscate, nedeteriorate. Produsele se vor livra la gratuit sediul UM 0771 Galați, str. Gării nr. 84 Oferta se va transmite până la data de 16.02.2026, ora 14:00, la adresa de e-mail: [email protected], semnată, ştampilată şi parolată. Parola se va transmite după 30 de minute de la data limită de depunere a ofertelor. Prețul va fi exprimat în lei, fără TVA, cu maxim 2 zecimale
