Denumirea completă a ofertantului - Sediul social al ofertantului - Codul unic de înregistrare - Reprezentantul legal al ofertantului, nume complet şi funcţia - Contul de trezorerie al ofertantului. 4.Certificat constatator actualizat emis de ONRC (din care să reiasă datele de identificare ale ofertantului şi obiectul de activitate autorizat specific contractului). 5. Modelul de contract asumat. 6. Declaratie privind neîncadrarea în prevederile referitoare la conflictul de interese. 7. Dupa stabilirea clasamentului ofertelor autoritatea contractanta va solicita prezentarea documentului care contine informatiile cu privire la beneficiarul real a fondurilor alocate din Programul Sanatate ofertantului clasat pe primul loc. Achizitia serviciilor se va face exclusiv online utilizand catalogul electronic al ofertantului declarat castigator. Ofertele transmise dupa data limita de depunere nu vor fi luate in considerare. Conform model de contract atasat Orice operator economic interesat va putea accesa documentaţia (caiet de sarcini si model contract) în format electronic la adresa www.e-licitatie.ro, secţiunea Publicitate Anunţuri. Eventualele solicitări de clarificări se vor trimite la adresa de e-mail [email protected] si [email protected] sau la Biroul Achiziţii UMF ,,Grigore T Popa” Iaşi, str Universităţii, nr 16, persoana de contact Daniel Vacariu-Sirotta, tel 0232/301863 ( în intervalul orar 8-16).
