Servicii de monitorizare dozimetrică individuală SERVICII DE MONITORIZARE DOZIMETRICA INDIVIDUALA Se solicită următoarele documente: Scrisoare de înaintare- Formular nr. 1; Certificat de înregistrare sau Certificat Constatator eliberat de Ministerul Justiției Oficiul Registrului Comerţului din care să reiasă că operatorul economic este legal constituit, că nu se află în niciuna dintre situațiile de anulare a constituirii precum și faptul că are capacitatea profesională fiind autorizat să realizeze activitățile care fac obiectul contractului; Propunerea tehnică - Declaraţie de consimţământ privind prelucrarea datelor cu caracter personal Formular nr.2; Propunerea financiară conținând Formularul de ofertă – Formular nr. 3 completat Declaraţie de acceptare a clauzelor contractuale - Formular nr. 4 Perioada de valabilitate a ofertei va fi de 120 zile de la depunerea acesteia. Încheierea contractului depinde de aprobarea bugetului, se va încheia contract lunar în cazul în care bugetul nu va fi aprobat. - Monitorizare dozimetrică a persoanelor expuse profesional la radiații ionizate (X și gamma); - Expedierea lunară a dozimetrelor necesare pentru monitorizarea dozimetrică individuală, înainte de data de 1 a fiecărei luni; - Dozimetrele sunt sigilate individual în punguțe cu etichete nominale; - Evaluarea și raportarea lunară a echivalentului de doza individual penetrant Hp(10) – valoare numerică, în termen de maxim 15 zile de la data primirii dozimetrelor purtate de la clienți; - Rapoarte de atenționare către client atunci când este depășită limita derivată de 1,7mSv/lună, datorate expunerii externe; - Rapoarte anuale conținând dozele cumulate pe perioada anului calendaristic; - Dozimetrele individuale sunt dozimetre reutilizabile și sunt proprietatea prestatorului. Se va încheia contract de prestări servicii valabil până la 31.12.2026. Ofertele se vor transmite arhivate cu parole până la data de 16.02.2026 ora 13:00, la adresa de e-mail: [email protected], semnate şi ştampilate. Parola pentru dezarhivarea ofertelor se va comunica prin e-mail în primele 30 minute de după ora limită stabilită pentru depunerea ofertelor. Formularele editabile vor fi transmise la cerere prin emailul mai sus menţionat
