Servicii medicale - investigatii medicale - BETA HCG Servicii medicale - investigatii medicale - BETA HCG - 1 serviciu - 100.00 lei fara TVA. Plata se va efectua in cont de TREZORERIE la 30 de zile de la data facturii si receptiei serviciului. Ofertele de pret se vor transmite la adresa de e-mail: [email protected], pana la data de 06.02.2026, prestarea serviciului se va efectua in baza solicitarii scrise transmisa de catre autoritatea contractanta. Documentele de calificare necesare, care vor însoți oferta, sunt: - dovada deținerii sediului sau punctului de lucru în Municipiul București sau județul Ilfov; -document edificator prin care să dovedească o formă de înregistrare, precum și faptul că are înscris în obiectul de activitate, o activitate corespunzătoare cu prestarea de servicii de analize medicale ce fac obiectul achizitiei publice ( certificat ONRC, certificat de înregistrare fiscală); - document care să dovedească autorizarea de funcționare pentru activitățile care fac obiectul contractului de achiziție publică, valabil la data limită de depunere a ofertelor ( autorizația sanitară de funcționare eliberată de Ministerul Sănătății-Direcția de Sănătate Publică); - document care atestă autorizarea si acreditarea laboratorului în care se vor presta serviciile ce fac obiectul achizitiei publice; -propunerea tehnico-financiară din care să rezulte tarifele aferente serviciului medical BETA HCG - Pentru informatii suplimentare desemnam ca persoana de contact pe domnisoara Andreea POPONEA, telefon 0745779217, 021 457.01.81 int. 252 fax: 457.01.54, e-mail: [email protected].
